Appointments and Office Hours

Our office is open to you from 9:00 am to 4:30 pm Monday thru Thursday and 9:00 am to 1:00 pm Fridays except for major holidays. Office visits are scheduled by appointment; please call 692-1400. Limited New Patient walk-in visits are available. Scheduled patients have priority. If you need to reschedule, please call at least 24 hours in advance so we may make your time available to another patient. Otherwise a fee may be assessed for a missed appointment.

New patients are important to us. To better serve you we request that all new patients arrive 20 minutes prior to the appointment to complete a personal health survey and other informational forms. Please bring copies of x-rays, EKG, lab work, and other pertinent material with you. This will enable us to avoid repeating costly tests and aid us in arriving at a diagnosis and treatment plan for you.

We know your time is valuable. Every effort will be made to see you as promptly as possible. Occasionally emergencies arise that cause unavoidable delay. We will be happy to see you as soon as possible or assist you in rescheduling your appointment.

Our commitment is to provide you with the best possible medical care. Our relationship is with you, our patient. All charges for service are ultimately your responsibility. Please present your insurance card at each visit. Copays must be collected at the time of your appointment. We will work with you to maximize your insurance benefits for needed services. We cannot guarantee payment or coverage by your insurer. We ask that you understand your insurance plan. Contact your insurer and your employer’s human resource department to clarify any concerns you may have regarding your health coverage benefits.
